Welcome to AIDS Community Resources North Country web site. The purpose of this web is to enhance the support services we provide to our clients and the community in general. We've provided a number of resources here to help you resolve problems and suggest improvements to our services.

AIDS Community Resources, through our staff and volunteers, exists to enhance the quality of life for people infected/affected by HIV/AIDS, facilitate access to available resources, end discrimination surrounding AIDS, prevent the further incidence of HIV infection throughought our nine county service region, and assist communities to address HIV/AIDS related needs in Northern, Central and Mohawk Valley Regions of New York State.
Information and referral:
ACR provides up-to-date information to individuals and the community to promote
awareness of AIDS intervention, prevention, resources, and services. The needs of clients
are met with individualized information and assistance obtaining needed services. The
needs of the community are addressed directly, and in coordination with other service
providers.
Services are provided to individuals dealing with an immediate problem which seriously disrupts his/her life and coping abilities. If you or someone you know is in a crisis contact your local ACR regional office or call the ACR hotline at 1-888-475-2437.
Everyone has the right to quality
services without discrimination. ACR is committed to ensuring access to entitlements and
services thereby assisting individuals in the development of greater self-sufficiency and
independence. ACR provides active assistance responding to individual needs regarding;
Housing, Medical, Educational and financial entitlements. Comprehensive Family Centered
Case Management. A case manager is assigned to each client to facilitate referrals and
provide coordination of services, ensuring that the clients receives appropriate service
in a timely manner. Together the case manager and client develop an individualized plan.
Case management services to be be arranged may include and are not limited to the
following: Support Groups, Medical/ Mental HealthCare, HIV/AIDS Education, Financial
Assistance,and Legal Assistance.
